How to apply

Start an application from the home page. You will enter your NIN, email, phone number, former name, and new name. Use the names exactly as they appear on your sworn affidavit.

Choose the reason for the change. Every file needs an affidavit. If the reason is marriage, upload the marriage certificate as well. Accepted files are PDF, JPG, or PNG, up to 10MB.

Review every field, confirm that the information is true, and submit. NameNotice issues an application reference in the form CNA-2026-XXXXXX. Use that number to track the file until it is published.
